Our client is seeking a LCSW/Case Manager/Substance abuse counselor in Brooklyn, Bronx, Queens, NY.

CLIENT'S REQUIRED SKILLS & EXPERIENCE
*Performs substance abuse assesments, collaborates with case managers to identify mutually agreed upon long and short-term susbtance abuse related goals and develop a comprehensive service plan.
*Must have a propensity and willingness to learn, persevere, and think outside the box.
*Complete HRA 2010E applications.
*Completes incident reports and submit to supervisor for reviews.
*Consistent and reliable.
*Train clients in activities of daily living skills.
*Service a case load of approximately 22 clients.
*Home visits and field work are required.
*Able to maintain healthy boundaries.
*Able to remain calm during moments of tension and stress.
*Strong written and verbal communication skills.
*Strong time-management, organizational skills, and attention to detail.
*Coordinate and supervise efforts to place new HASA clients in vacated units within a maximum of 30 days from referral.
*Coordinate and supervise efforts with case management and maintenance staff to ensure a minimum monthly occupancy rate of 95% is maintained;
*Provide training to new and current case managers;
*Ensures that the home visit policy and procedures is enforced, tracked and documented.
*Performs Audits of client charts to ensure comprehensive services planning and accurate charting of client progress;
