Job Description

WHO WE ARE : The Children’s Home of Wyoming Conference partners with children, families and communities throughout New York State to inspire hope, develop skills and cultivate healthy relationships for positive futures.

Position Summary:  The initial role of the Healthy Relationship Coach will be to develop honest, trusting relationships with families. The Healthy Relationship Coach will utilize strength-based assessments and risk/harm reductions strategies to develop an action-oriented service plan-addressing the current and future factors contributing to referrals. The Healthy Relationship Coach will work in collaboration with the assigned DSS caseworker to establish family goals, outcomes, and markers for success.

Responsibilities:

  • Conduct intake, assessments, and develop service plans
  • Develop relationships with families using motivational interviewing techniques
  • Support families in implementing plans, meeting and communicating with families according to established guidelines, assessing risk factors and developing strategies to manage those factors, scheduled new appointments and follow-up visits
  • Communicating case contact with county workers, CHWC supervisors, and case collaterals
  • Record and document all case interactions and activities

Requirements

Education:

  • High School Diploma / GED  required

Experience:

  • 1+ years experience working with youth and families  required
  • Driver's License and ability to maintain insurability throughout employment  required
