Job Description

We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality patient care in the comfort of their own homes. This role is ideal for nurses who value independence and enjoy building relationships with patients and their families. Our Home Health team is dedicated to delivering personalized care that improves health outcomes and enhances the overall well-being of our patients. Duties:

• Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting vital signs assessments.
• Develop and implement individualized care plans in collaboration with patients, their families, and other healthcare professionals.
• Collaborate with primary care physicians and other healthcare providers to ensure seamless care transitions.
• Educate patients and their families on disease management, health coaching, and family planning.
• Conduct discharge planning to ensure patients are adequately prepared for hospital discharge or transition to long-term care facilities.
• Utilize medical records to identify patient needs and develop targeted treatment plans.
• Administer injections and perform catheter care as needed.
• Provide critical care to patients with complex medical conditions.
• Utilize internal medicine knowledge to diagnose and treat patients with chronic health conditions.
• Monitor patient progress and adjust treatment plans as necessary.

Skills:

• Proven experience as a Registered Nurse with a strong background in home health care.
• Experience administering injections.
• Knowledge of family planning and health coaching principles.
• Strong critical care skills, with the ability to assess and manage complex patient conditions.
• Proficiency in medical records management and documentation.
• Ability to take and record vital signs accurately and efficiently.
• Experience with catheter care and maintenance.
• Familiarity with internal medicine and the ability to apply this knowledge in a home health setting.
