Job Summary Full Time Day 8am - 4:30pm Monday - Friday; REQUIRES: High School Diploma or GED and 2 years related experience; prior neurology or patient navigation experience preferred Job Duties Coordinates patient care and maximizes patient satisfaction and experience. Partners with care providers to serve as a liaison to ensure effective communication with patients. + Provides discharge information. + Partners with providers to ensure a smooth transition at discharge to home or other facilities as required. + Ensures additional resources are made available for the patient as needed. + Effectively collaborates with other healthcare providers and non-Geisinger employees in coordinating and implementing the patients plan of care. + Maintains systems and processes to assure timely follow-up and surveillance of a defined group of patients. + Serves as liaison with clinical and administrative personnel. + Establishes point of contact for referring providers to create immediate access to provider templates. + Develops and implements efficient patient navigational procedures. + Investigates patient concerns including billing, appointment access, perception of care, physician issues and communication problems. + Develops, maintains, and audits standard operating procedures of patient access and utilization process. + May provide administrative support as needed.
