Silver Spring Healthcare Services, located in Spring, TX, is seeking a compassionate and skilled LVN/RN to join our team as a Home Health LVN/R on a PRN basis. Responsibilities:

• Perform in-home patient assessments and customize care plans based on individual needs.
• Administer prescribed medications and treatments following healthcare providers' instructions.
• Regularly monitor and document patient vital signs and progress to track improvements.
• Educate patients and their families on disease management and ways to promote overall well-being.
• Work collaboratively with diverse healthcare teams to ensure effective coordination of patient care.

Qualifications:

• Must have at least 2 years of experience working in Private duty nursing as an LVN/RN in Spring, TX, US.
• Required to possess an active LVN or RN license, specifically in the state of Texas.
• Proficient in the performance of patient assessments and the development of personalized care plans.
• Strong communication skills are essential, both verbal and written.
• Capable of working autonomously and collaboratively within a multidisciplinary healthcare team.
