Key Responsibilities:

Conduct in-home health assessments to evaluate patients' needs.

Collaborate with healthcare teams to ensure continuity of care.
What You'll Need:

RN License: Current New York state nursing license.
Experience: At least 2 years of nursing experience, ideally in home care or community health settings.
UAS Certification: Certification or willingness to obtain UAS certification.
Location: This role is based in The Bronx, New York.
